Vitamin B12 Deficiency: Why Older Adults and Vegans Are Most at Risk

A Common Deficiency With Major Consequences

Vitamin B12 deficiency is one of the most underdiagnosed nutritional deficiencies in adults. Estimates suggest 6 percent of adults under 60 and 20 percent of adults over 60 have low or borderline B12 levels — yet many remain undiagnosed for years.

The consequences range from fatigue and cognitive symptoms to severe, sometimes irreversible neurologic damage. The good news: identification is straightforward when you look for it, and treatment is highly effective.

What B12 Does

B12 (cobalamin) is essential for:

  • DNA synthesis (especially in rapidly dividing cells like blood cells)
  • Red blood cell production
  • Neurologic function — both the central nervous system and peripheral nerves
  • Methylation reactions throughout the body

Deficiency affects all of these processes, but neurologic damage is the most concerning because it can become irreversible.

Recognizing the Symptoms

B12 deficiency causes a wide range of symptoms:

Hematologic

  • Fatigue
  • Weakness
  • Pale skin
  • Shortness of breath
  • Macrocytic anemia (large red blood cells)

Neurologic (often the first sign)

  • Numbness and tingling in hands and feet
  • Difficulty walking, balance problems
  • Loss of position sense
  • Memory problems, brain fog
  • Mood changes — depression, irritability
  • Confusion in older adults
  • In severe cases: weakness, gait abnormalities, dementia-like symptoms

Other

  • Glossitis (inflamed, smooth, beefy red tongue)
  • Mouth ulcers
  • Loss of appetite
  • Weight loss

A critical point: neurologic symptoms can occur without anemia. Patients with normal blood counts may still have significant B12 deficiency causing nerve damage.

Who Is at Risk

Reduced Absorption

The most common cause of B12 deficiency in adults is reduced absorption, not low intake:

  • Pernicious anemia — autoimmune destruction of stomach cells that produce intrinsic factor
  • Atrophic gastritis — common in older adults; reduces stomach acid needed for B12 release
  • Long-term PPI use or H2 blockers — reduces stomach acid
  • Long-term metformin use — common in diabetes; affects B12 absorption
  • Bariatric surgery — particularly gastric bypass and gastrectomy
  • Inflammatory bowel disease affecting the terminal ileum
  • Celiac disease

  • Pancreatic insufficiency
  • Older age (decreased stomach acid is common)

Inadequate Intake

  • Vegans — B12 is found almost exclusively in animal products
  • Strict vegetarians with limited dairy and egg intake
  • Older adults with poor appetite or limited food variety
  • Alcoholism with poor nutrition

Increased Needs

  • Pregnancy and lactation
  • Hyperthyroidism
  • Some chronic illnesses

Diagnostic Testing

Total B12 Level

The basic test is total serum B12. Interpretation:

  • > 400 pg/mL: Generally normal
  • 200–400 pg/mL: Borderline — may have functional deficiency despite "normal" level
  • < 200 pg/mL: Likely deficiency

The reference range may differ slightly between labs.

When the Total B12 Is Borderline

If symptoms or risk factors suggest deficiency despite a borderline-normal B12 level, additional testing helps:

  • Methylmalonic acid (MMA) — elevated in true B12 deficiency; more sensitive than total B12
  • Homocysteine — elevated in B12 deficiency (also folate deficiency); less specific
  • Holotranscobalamin (active B12) — measures the metabolically active form; not yet widely available

These metabolic markers are particularly valuable when symptoms are suggestive but B12 level is borderline.

Identifying the Cause

Once deficiency is confirmed:

  • Intrinsic factor antibodies — positive in pernicious anemia
  • Parietal cell antibodies — supports autoimmune gastritis
  • CBC with peripheral smear — looking for macrocytic anemia, hypersegmented neutrophils
  • Folate level — folate deficiency can mask B12 deficiency on smear
  • Sometimes upper endoscopy if atrophic gastritis is suspected
  • Sometimes Schilling test (rarely used now)

Treatment

Acute Replacement

For symptomatic deficiency:

  • Intramuscular B12 injections — typically 1000 mcg daily for a week, then weekly for several weeks, then monthly
  • High-dose oral B12 — 1000–2000 mcg daily; equally effective for most patients with absorption issues

Once thought to be necessary only by injection in patients with absorption problems, high-dose oral B12 has been shown to work well even in pernicious anemia. This is because passive absorption (about 1 percent) doesn't require intrinsic factor, and at high doses, that 1 percent is still enough to meet daily needs.

Maintenance

After initial replacement:

  • Patients with absorption problems: lifelong supplementation
  • Patients with dietary causes: dietary correction or supplementation as long as the dietary issue persists
  • Patients with reversible causes (e.g., metformin started recently): periodic monitoring

Vegans and Vegetarians

  • Daily supplementation (250–1000 mcg) is generally recommended for strict vegans
  • Fortified foods (some plant milks, nutritional yeast, fortified cereals) help
  • Periodic level monitoring is reasonable

Older Adults

The Institute of Medicine recommends adults over 50 obtain B12 from synthetic sources (supplements or fortified foods) because of high rates of impaired absorption from food-bound B12.

Recovery Timeline

  • Hematologic improvements occur within weeks
  • Neurologic improvements may take months and can be incomplete if deficiency was prolonged
  • The longer neurologic deficits persist before treatment, the less complete recovery tends to be

This is why early recognition matters — delaying treatment of B12 deficiency causing neurologic symptoms risks permanent damage.

Common Misconceptions

  • "My B12 was tested and was normal": Total B12 in the borderline range may still represent functional deficiency in symptomatic patients
  • "I take a multivitamin so I can't be deficient": Multivitamins typically contain low-dose B12 that may not overcome absorption issues
  • "Only vegans get B12 deficiency": Older adults and patients with absorption issues are at greater risk
  • "I need monthly injections to absorb B12": High-dose oral B12 is effective for most patients

When to See Your Doctor

  • New numbness, tingling, or balance problems
  • Persistent fatigue without clear cause
  • Memory problems or brain fog
  • Macrocytic anemia found on routine labs
  • Long-term metformin or PPI use without recent B12 testing
  • Vegan or strict vegetarian diet without supplementation
  • Family history of pernicious anemia
  • Older age (over 60–65) without recent screening
