Long-Term PPI Use: The Risks Your Doctor Should Discuss

A Common Medication With Important Long-Term Considerations

Proton pump inhibitors (PPIs) — including omeprazole (Prilosec), esomeprazole (Nexium), pantoprazole (Protonix), lansoprazole (Prevacid), and rabeprazole — are among the most prescribed medications in the world. They are highly effective for acid-related conditions and remarkably safe in short-term use.

The trouble is that many patients take PPIs for years or decades, often without a clear ongoing indication. Long-term use carries real risks that deserve discussion. When PPIs Are Clearly Indicated Long-Term

Several conditions have strong evidence for indefinite or long-term PPI therapy:

  • Erosive esophagitis that has healed with PPI therapy
  • Barrett's esophagus
  • Severe GERD that recurs immediately when PPIs are stopped — see our GERD vs. heart attack article for symptom comparison
  • History of bleeding ulcer (peptic ulcer disease)
  • Zollinger-Ellison syndrome (gastrin-secreting tumor)
  • Chronic NSAID use in patients at high risk of ulcers
  • Eosinophilic esophagitis (in some patients)
  • High-dose prevention in patients on dual antiplatelet therapy with risk factors

For these patients, the benefit of continued therapy clearly outweighs the risks.

When PPIs Often Get Continued Without Clear Indication

Many patients end up on long-term PPIs through pathways like:

  • Started in the hospital for stress ulcer prophylaxis and never stopped
  • Started for vague "indigestion" or non-ulcer dyspepsia
  • Continued years after H. pylori was successfully treated
  • Started during an NSAID course that was discontinued long ago
  • Started for mild reflux that could be managed with lifestyle measures

These patients deserve a re-evaluation: is the original indication still present, and could lifestyle measures or step-down therapy work?

Real Risks of Long-Term Use

Several risks have substantial supporting evidence:

Vitamin B12 Deficiency

PPIs reduce gastric acid, which is needed to release vitamin B12 from food proteins. Long-term use can cause B12 deficiency, particularly in older adults. Periodic checking is reasonable for patients on PPIs for more than 2 years.

Magnesium Deficiency

Long-term PPI use can cause significant hypomagnesemia, sometimes requiring IV repletion. Symptoms include muscle cramping, fatigue, arrhythmias, and seizures in severe cases. Annual magnesium checks are reasonable for long-term users.

Bone Fractures

PPIs are associated with a modest increase in hip, wrist, and spine fractures. The mechanism likely involves reduced calcium absorption and possibly direct effects on bone. The risk is most pronounced in patients with other osteoporosis risk factors.

C. difficile Infection

Acid suppression allows C. diff to survive passage through the stomach more easily. Long-term PPI use modestly increases C. diff infection risk, particularly in patients also receiving antibiotics.

Pneumonia

Some studies show modestly increased rates of community-acquired pneumonia in PPI users, possibly through bacterial overgrowth in the upper GI tract.

Kidney Effects

Long-term PPI use is associated with a modest increase in chronic kidney disease and acute interstitial nephritis. The absolute risk is small but real.

Iron Deficiency

Acid is helpful for iron absorption. Long-term PPI users — particularly menstruating women, vegetarians, and those with marginal intake — may develop iron deficiency. See our iron deficiency in women guide.

Risks That Are Likely Overstated

Some publicized risks have weaker evidence:

  • Dementia: Initial concerning studies have not been consistently confirmed; the relationship is uncertain
  • Heart attack risk: Largely refuted by subsequent research
  • Most cancers: No clear evidence of causation, though associations exist with some upper GI cancers (likely confounded)

It is reasonable to take a balanced view: PPIs are not innocuous, but they are not dangerous medications either when used appropriately.

Lifestyle Measures Often Underused

Before settling for indefinite PPI therapy for mild GERD, several measures help substantially:

  • Weight loss in patients with overweight or obesity
  • Avoiding meals within 3 hours of bedtime
  • Elevating the head of the bed by 6–8 inches (or using a wedge)
  • Avoiding personal trigger foods — common triggers include alcohol, chocolate, peppermint, very fatty meals, citrus, tomato-based foods, and caffeine
  • Smaller, more frequent meals
  • Smoking cessation
  • Reducing alcohol intake

These measures alone control mild reflux for many patients.

How to Deprescribe Safely

Stopping PPIs cold turkey often causes rebound acid hypersecretion — temporarily worse symptoms that can be mistaken for the original problem returning. A planned approach reduces this:

Step 1: Assess Whether Continued Therapy Is Needed

For patients on PPIs for non-essential indications:

  • Confirm there is no history of bleeding ulcer, severe esophagitis, or Barrett's esophagus
  • Confirm symptoms are mild and well-controlled
  • Discuss the goal of trying to reduce or stop

Step 2: Step Down Before Stopping

Several effective approaches:

  • Reduce from twice daily to once daily for 2–4 weeks
  • Switch from daily to every other day for 2–4 weeks
  • Switch to an H2 blocker (famotidine) which can be tapered more easily
  • Use as-needed PPI rather than daily

Step 3: Anticipate Rebound

A short period of worse symptoms when stopping is normal — typically 2–4 weeks — and does not necessarily mean the underlying disease is uncontrolled. Antacids (Tums, Maalox), an H2 blocker as needed, or alginates (Gaviscon) can manage this transition.

Step 4: Have a Plan for Recurrence

If symptoms return:

  • Try lifestyle modifications first
  • Use as-needed therapy (H2 blocker or PPI)
  • Restart daily PPI only if needed and if specific indications justify it

Special Situations

Patients with Frequent NSAID Use

If you take regular NSAIDs and have ulcer risk factors (older age, prior ulcer, anticoagulant use, high NSAID dose), continued PPI therapy may remain appropriate.

Older Adults on Multiple Medications

Long-term PPI use should be a specific topic during medication reviews — see our article on polypharmacy.

Patients with Atypical or Concerning Symptoms

Symptoms like difficulty swallowing, food getting stuck, unintentional weight loss, persistent vomiting, blood in stool, or new severe heartburn after age 50 warrant endoscopic evaluation rather than continued empirical PPI therapy.

When to See Your Doctor

  • You have been on a PPI for over a year and have not discussed whether to continue
  • New or worsening symptoms despite PPI therapy
  • Difficulty swallowing, weight loss, or other concerning features
  • Considering trying to come off your PPI
  • Need to address vitamin or mineral deficiencies that may be related
