The "Skipped Beat" That Isn't Really Skipped
Premature ventricular contractions (PVCs) are extra heartbeats that originate in the ventricles — the heart's lower pumping chambers — instead of the normal pacemaker site in the right atrium. They are the single most common cause of the "skipped beat" sensation patients describe.
Despite how alarming they can feel, PVCs are extremely common: studies show that essentially everyone has occasional PVCs, and roughly half of adults have them caught on a 24-hour Holter monitor. Most are completely benign. Some, however, deserve attention.

What's Actually Happening
A normal heartbeat originates in the sinus node at the top of the right atrium, travels through the AV node, and reaches the ventricles. A PVC starts somewhere in the ventricles and "fires" early, before the next normal beat. The ventricles then pause briefly to reset, and the next normal beat comes in stronger because the heart has had extra time to fill — that "extra strong" beat is what most patients feel as the "thump."
What patients describe as a "skipped beat" is actually an extra beat plus a compensatory pause. The heart didn't skip; it fired one too soon and then waited.
When PVCs Are Almost Certainly Benign
Reassuring features include:
- Occasional PVCs in an otherwise healthy person
- Normal heart structure on echocardiogram
- No family history of sudden cardiac death
- No symptoms of heart failure
- Symptoms triggered by clear factors (caffeine, stress, lack of sleep)
- Normal heart function and ECG between episodes
Most healthy adults can be reassured after a basic evaluation.
When PVCs Need More Attention
Several features push toward more thorough workup:
- High burden — PVCs comprising more than 10–15 percent of total beats over 24 hours
- Multifocal PVCs — coming from multiple sites in the ventricle
- Couplets, triplets, or runs of PVCs — strings of PVCs in a row (non-sustained ventricular tachycardia)
- R-on-T phenomenon — PVCs occurring during the vulnerable period of the previous beat
- Symptoms with exercise — exercise-induced PVCs are more concerning
- History of structural heart disease — prior heart attack, cardiomyopathy, valvular disease
- Family history of sudden cardiac death
- Symptoms suggestive of heart failure — shortness of breath, swelling, fatigue
The PVC-Induced Cardiomyopathy Phenomenon
A key reason to quantify PVC burden: very frequent PVCs (typically more than 10–20 percent of total beats) over months to years can cause the heart muscle to weaken — a condition called PVC-induced cardiomyopathy. The good news is that this cardiomyopathy is usually reversible if the PVCs are eliminated, typically with catheter ablation. The danger is that this can develop without dramatic symptoms.
This is why a 24-hour Holter or extended monitor is essential when patients report frequent PVCs — to determine the actual burden, not just that they exist.
Standard Workup
For patients with PVCs:
- Detailed history — frequency, triggers, associated symptoms, family history
- Physical exam — heart, lungs, signs of heart failure
- 12-lead ECG — captures PVC morphology when present
- Holter or extended monitor — quantifies burden over 24 hours to 30 days
- Echocardiogram — assesses heart structure and function
- Basic labs — electrolytes (potassium, magnesium), TSH, sometimes others
In selected patients:
- Stress test — particularly if exercise worsens symptoms
- Cardiac MRI — when structural disease is suspected but echocardiogram is normal
- Electrophysiology referral — when ablation is being considered
Modifiable Triggers
Many patients see substantial improvement with simple changes:
- Reduce or eliminate caffeine — coffee, tea, energy drinks, pre-workout supplements
- Limit alcohol — even moderate intake can trigger PVCs in sensitive individuals
- Avoid stimulant medications when possible — pseudoephedrine, ADHD medications, weight loss supplements
- Improve sleep — sleep deprivation reliably worsens PVCs
- Manage stress and anxiety
- Stay hydrated — particularly important in Florida heat
- Optimize electrolytes — particularly magnesium and potassium
- Treat sleep apnea when present
Medical Treatment
When PVCs are symptomatic or burden is high:
- Beta blockers (metoprolol, atenolol) — first-line for symptomatic PVCs
- Calcium channel blockers (verapamil, diltiazem) — for certain PVC patterns
- Antiarrhythmic medications (flecainide, sotalol) — for selected patients under cardiology guidance
Most patients with mild symptoms and low PVC burden need no specific medication.
Catheter Ablation
For patients with high PVC burden, symptomatic PVCs not controlled by medication, or PVC-induced cardiomyopathy, catheter ablation is often curative. The procedure maps the origin of the PVCs and uses radiofrequency energy or cryotherapy to eliminate the focus. Success rates exceed 80–90 percent for typical PVC patterns.
What Most Patients Need to Hear
For most patients with PVCs:
- They are extremely common
- They do not predict heart attack or sudden death in healthy people
- Symptoms can usually be reduced significantly with trigger modification
- A Holter monitor and echocardiogram give a complete enough picture for the vast majority
- Treatment is reserved for those with high burden or significant symptoms
