Frozen Shoulder: Why It Happens and How to Get Moving Again

A Slow-Motion Shoulder Problem

Frozen shoulder, formally called adhesive capsulitis, is one of the most frustrating shoulder conditions in medicine. It develops gradually, peaks with severe pain and dramatic stiffness, then slowly resolves over a year or more — often with surprisingly limited treatment options that change the timeline.

It is not a rotator cuff tear, not arthritis, and not a pinched nerve. The shoulder capsule itself becomes inflamed and contracted, restricting motion in every direction.

Who Develops It

Strong risk factors include:

  • Diabetes — risk is 2–4 times higher and the disease tends to be more severe and bilateral
  • Thyroid disease (both hyper- and hypothyroidism)
  • Female sex — women are 2–3 times more affected
  • Age 40 to 60 — uncommon outside this window
  • Recent shoulder immobilization — after a fracture, surgery, or stroke
  • Parkinson's disease and certain cardiac conditions

A significant percentage of cases are "idiopathic" — meaning we never identify a clear trigger.

The Three Phases

Recognizing which phase you are in shapes treatment expectations:

Phase 1: Freezing (2–9 months)

Gradual onset of shoulder pain, often worse at night, that limits motion progressively. This is the most painful phase. Sleeping on the affected side becomes impossible. Reaching behind the back for a wallet or to fasten a bra is difficult.

Phase 2: Frozen (4–12 months)

Pain may decrease somewhat, but stiffness becomes the dominant problem. External rotation (turning the arm out away from the body) is typically the most restricted direction — a key diagnostic feature. Daily activities like washing hair, putting on a coat, and reaching overhead become difficult or impossible.

Phase 3: Thawing (5–24 months)

Slow, gradual return of motion. Pain continues to decrease. Most patients eventually regain near-normal motion, though some have permanent mild restriction.

The total course typically runs 18 to 30 months without aggressive intervention. With treatment, motion can return faster, but the underlying biology of the condition limits how much treatment can compress the timeline.

Diagnosis Is Mostly Clinical

The history (gradual painful stiffness without trauma) and exam (loss of both active and passive range of motion, particularly external rotation) make the diagnosis. X-rays are often obtained to rule out arthritis but are usually normal in frozen shoulder. MRI is rarely needed unless the diagnosis is unclear or rotator cuff pathology is suspected.

Treatment Options That Help

Pain Control First

In the freezing phase, controlling pain is the priority. Options include:

  • Acetaminophen and NSAIDs (with attention to GI and kidney risks)
  • Sleep support, since night pain is a major driver of fatigue and mood decline
  • Ice or heat — patient preference

Corticosteroid Injection

Intra-articular corticosteroid injection is one of the most effective interventions, particularly when given early in the freezing phase. Studies show meaningful improvement in pain and function within weeks. The benefit appears greatest when combined with physical therapy.

Physical Therapy

Gentle range-of-motion exercises performed daily — but never aggressively — help maintain function. The classic mistake is pushing too hard, too fast: this typically increases inflammation and worsens pain without speeding recovery. A skilled physical therapist tailors progression to the phase you are in.

In the frozen phase, stretching is more aggressive and can be uncomfortable. Patient education is essential: pain during gentle stretching is acceptable; sharp or worsening pain afterward is not.

Hydrodilatation

A procedure where the shoulder joint is injected with a large volume of saline (often combined with corticosteroid and local anesthetic) to physically distend and stretch the capsule. Done under ultrasound guidance, it offers another effective option for stubborn cases without going to surgery.

Surgical Options

For shoulders that fail extensive conservative care after 6–12 months, options include:

  • Manipulation under anesthesia (MUA) — the surgeon moves the shoulder through full range while you are anesthetized to break up adhesions
  • Arthroscopic capsular release — the contracted capsule is cut surgically, providing immediate motion improvement

Both are followed by intensive physical therapy. The American Academy of Orthopaedic Surgeons considers these options after at least 6 months of conservative treatment.

What Doesn't Work

Several common attempts have little evidence behind them:

  • Heat lamps or ultrasound therapy alone
  • Acupuncture as a standalone treatment
  • Vigorous massage of the shoulder
  • "Forcing" range of motion with weights or pulleys early in the disease

Diabetes Changes the Equation

Patients with diabetes — particularly poorly controlled — have:

  • Worse pain
  • Greater functional limitation
  • Higher rates of bilateral disease
  • Poorer response to corticosteroid injections
  • Higher recurrence rates

Optimizing blood sugar control matters, both for shoulder outcomes and to prevent recurrence on the opposite side.

Setting Expectations

Frozen shoulder is one of the few common conditions where time is genuinely the dominant factor. Treatment helps with symptoms and may speed recovery somewhat, but a 12–18 month timeline is realistic for most patients. Knowing this helps patients pace expectations and avoid the discouragement that comes from expecting weeks-long recovery.
