A Common, Treatable Cause of Sleep Disruption
Restless legs syndrome (RLS), also called Willis-Ekbom disease, affects an estimated 5–10 percent of adults. It causes uncomfortable sensations in the legs and an irresistible urge to move them, particularly when sitting still or lying down. The result is significant sleep disruption, daytime fatigue, and reduced quality of life.
RLS is frequently misdiagnosed, partly attributed to anxiety or "leg cramps," and often inadequately treated. The good news: with the right diagnosis and approach, most patients improve substantially.

The Diagnostic Criteria
The International Restless Legs Syndrome Study Group criteria require all five:
- An urge to move the legs, usually accompanied by uncomfortable sensations
- Symptoms begin or worsen during periods of rest or inactivity
- Symptoms are partially or totally relieved by movement
- Symptoms are worse in the evening or at night
- Symptoms are not better explained by another condition (leg cramps, positional discomfort, arthritis, leg edema)
Patients describe the sensations as crawling, tingling, pulling, aching, or "antsy" — often hard to put into words.
What RLS Is Not
Several conditions can mimic RLS:
- Nocturnal leg cramps — sudden painful muscle contractions, not relieved by movement
- Peripheral neuropathy — burning or numbness without urge to move
- Akathisia — generalized inner restlessness, often medication-related
- Anxiety with motor restlessness — generalized rather than leg-focused
- Periodic limb movement disorder — limb movements during sleep without daytime urge to move
A careful history distinguishes these.
Periodic Limb Movements of Sleep
Many RLS patients also have periodic limb movements of sleep (PLMS) — repetitive leg movements during sleep that the patient often doesn't recognize but that disturb sleep quality. Bed partners often notice these. Sleep studies can document them but aren't required for RLS diagnosis.
Why It Often Goes Undiagnosed
- Patients describe symptoms in terms hard to recognize as RLS
- Symptoms attributed to "just bad sleep" or stress
- Worsens with age but treated as normal aging
- Iron studies rarely checked despite being central to evaluation
- Treatment targeted at sleep rather than underlying disorder
Causes and Associations
Primary RLS
- Idiopathic
- Strong genetic component — roughly 50 percent of patients have a family history
- Multiple identified gene variants
Secondary RLS
Several conditions trigger or worsen RLS:
- Iron deficiency — central to RLS pathophysiology; even normal-range iron levels can be inadequate
- Pregnancy — particularly third trimester
- End-stage kidney disease
- Diabetes
- Peripheral neuropathy
- Multiple sclerosis
- Parkinson's disease

- Folate deficiency
Medications That Worsen RLS
- Most antidepressants (SSRIs, SNRIs, mirtazapine, tricyclics)
- Antihistamines (especially diphenhydramine)
- Antinausea medications (metoclopramide, prochlorperazine)
- Dopamine antagonist antipsychotics
Reviewing medications often reveals contributors.
The Critical Role of Iron
Iron is essential for dopamine production in the brain, and brain iron deficiency drives RLS even when systemic iron levels appear adequate.
Iron Studies in RLS
Standard testing:
- Ferritin — most important; should be > 75 ng/mL in RLS patients (target higher than typical "normal" labs)
- Transferrin saturation — should be > 20 percent
- Sometimes total iron, TIBC, soluble transferrin receptor
Many RLS patients with normal-range ferritin have subclinical iron insufficiency for RLS purposes. The 75 ng/mL target is specific to RLS, not general iron health.
Iron Repletion
- Oral iron (ferrous sulfate, ferrous gluconate) — first-line
- Take with vitamin C, away from coffee/tea/calcium
- Every-other-day dosing improves absorption and reduces GI side effects
- IV iron for patients with poor oral absorption or insufficient response
- Recheck ferritin in 8–12 weeks
Iron repletion alone resolves or substantially improves RLS in many patients.
Lifestyle Measures
For mild RLS or as adjuncts to medication:
- Regular moderate exercise (intense exercise close to bedtime can worsen)
- Avoid caffeine, particularly in evening
- Limit alcohol
- Address sleep hygiene
- Warm or cool baths before bed (individual preference)
- Leg massage, stretching
- Pneumatic compression devices (commercial RLS units exist)
- Identify and address aggravating medications when possible
Medical Treatment
The treatment landscape has shifted significantly. Newer guidelines favor different approaches than 10–15 years ago.
Alpha-2-Delta Ligands (Now First-Line)
- Gabapentin enacarbil (Horizant) — FDA-approved for RLS
- Gabapentin — off-label, widely used, more affordable
- Pregabalin — also effective
These medications have largely replaced dopamine agonists as first-line for many patients due to lower long-term complication rates. Side effects include sedation, dizziness, weight gain, and edema.
Dopamine Agonists
- Pramipexole, ropinirole, rotigotine patch
- Historically first-line; effective short-term
- Major problem: augmentation — paradoxical worsening of RLS with continued use, with symptoms occurring earlier in the day, more severely, and in additional body parts
- Augmentation affects up to 70 percent of patients on dopamine agonists long-term
- Now usually reserved for selected cases or shorter-term use
- Impulse control disorders (gambling, hypersexuality, compulsive shopping) are recognized side effects
If a patient is on a dopamine agonist long-term and developing augmentation, transition to alpha-2-delta ligands or low-dose opioids is often needed.
Opioids
- Low-dose oxycodone, methadone, or other opioids effective for severe refractory RLS
- Reserved for severe cases failing other treatments
- Risk-benefit considerations including dependence and other opioid-related concerns
Other Options
- Benzodiazepines (clonazepam) — sometimes used short-term
- IV iron for selected severe cases
Sleep Considerations
Beyond RLS treatment, address general sleep:
- Address coexisting sleep apnea — see our sleep apnea guide

- Address shift work or schedule issues
- Treat coexisting depression or anxiety carefully (medication choices matter)
Pregnancy Considerations
RLS commonly worsens during pregnancy, particularly in the third trimester. Approach:
- Iron studies and iron repletion as needed (often adequate)
- Folate supplementation
- Lifestyle measures
- Most RLS medications are limited or avoided in pregnancy
- Symptoms usually improve after delivery

- Iron deficiency drives a significant portion of cases
- Treatment landscape has shifted away from dopamine agonists for most patients
- Augmentation from dopamine agonists is a major concern

- Persistent uncomfortable leg sensations affecting sleep
- Difficulty sitting still in the evening
- Bed partner observations of leg movements
- Family history of RLS plus symptoms
- Iron deficiency or anemia
- Medications that may be contributing
- Currently treated RLS that is worsening (consider augmentation)
