A Leading Cause of Central Vision Loss
Age-related macular degeneration (AMD) is the leading cause of severe vision loss in adults over 50 in developed countries. The macula — the small central area of the retina responsible for sharp central vision and color perception — gradually deteriorates, affecting reading, driving, recognizing faces, and detail vision.
Unlike many eye conditions, AMD has clear risk factors, established prevention strategies, and effective treatment for advanced wet AMD. Yet many patients first learn about AMD only after significant vision has already been lost.

The Two Main Types
Dry AMD (Atrophic)
- Accounts for 85–90 percent of AMD cases
- Gradual deterioration of the macula
- Progresses slowly over years
- Currently no FDA-approved treatment to reverse it (though new options are emerging)
- Can progress to advanced stage with significant central vision loss
- Some patients with dry AMD develop wet AMD
Wet AMD (Neovascular or Exudative)
- Accounts for 10–15 percent of AMD but causes most of the rapid vision loss
- Abnormal blood vessels grow under the retina
- These vessels leak blood and fluid, damaging the macula
- Can cause rapid vision loss over weeks to months
- Treatable with intravitreal injections that have transformed outcomes
Recognizing Symptoms
Common features of AMD:
Early Symptoms
- Often no symptoms in early disease (detected on routine exam)
- Slight blurring of central vision
- Difficulty adjusting between bright and dim light
- Need for brighter light to read or do detail work
- Decreased color brightness
More Advanced Symptoms
- Distortion of straight lines (door frames, lines on paper appear wavy)
- Dark or empty area in central vision
- Difficulty recognizing faces
- Difficulty reading
- Decreased ability to perceive detail
- Peripheral vision usually preserved
The Amsler grid is a simple at-home test where lines that appear bent or sections that appear missing suggest macular changes warranting prompt evaluation.
Who Is at Risk
Strongest Risk Factors
- Age — risk rises substantially after 55
- Family history — having a first-degree relative with AMD significantly increases risk
- Smoking — increases risk 2–4 fold and accelerates progression; the most important modifiable risk factor
- Light skin/eyes
- European ancestry
Other Risk Factors
- Female sex (slightly higher prevalence)
- Cardiovascular disease
- Hypertension
- Obesity
- Sun exposure (less established but plausible)
- Certain dietary patterns
Genetic Testing
Several gene variants influence AMD risk significantly. Genetic testing is available but not routinely recommended outside of research contexts because results don't currently change management substantially.
Diagnostic Evaluation
Standard AMD evaluation:
- Visual acuity testing
- Amsler grid testing
- Dilated eye examination
- Optical coherence tomography (OCT) — detailed cross-sectional imaging of the macula; the cornerstone of modern AMD evaluation
- Fluorescein angiography or OCT angiography — used to identify abnormal blood vessels in suspected wet AMD
- Fundus photography — documentation and tracking
OCT has revolutionized AMD detection and monitoring. It can identify subtle changes years before symptoms develop and is critical for monitoring response to treatment.
Treatment for Dry AMD
AREDS2 Supplements
Based on the Age-Related Eye Disease Study (AREDS) and follow-up AREDS2:
The AREDS2 formulation:
- Vitamin C 500 mg
- Vitamin E 400 IU
- Lutein 10 mg
- Zeaxanthin 2 mg
- Zinc 80 mg (or 25 mg for some formulations)
- Copper 2 mg
Important findings:
- AREDS2 supplements reduce progression to advanced AMD by about 25 percent in patients with intermediate AMD or advanced AMD in one eye
- They do NOT prevent AMD in patients without it
- They do NOT help patients with only mild AMD
- Beta-carotene from the original formula was removed in AREDS2 because of lung cancer risk in current and former smokers
This is one of the rare circumstances where specific supplementation has clear evidence of benefit — but only for the right patients.
Lifestyle Measures
- Smoking cessation is the single most impactful intervention
- Mediterranean-style diet — multiple studies show benefit
- Diet high in dark leafy greens (kale, spinach) and colorful vegetables
- Fish high in omega-3 (salmon, sardines) — twice weekly may help
- Cardiovascular health optimization — blood pressure, lipids, glucose

- Regular monitoring of central vision with Amsler grid
Emerging Treatments
- Pegcetacoplan (Syfovre) and avacincaptad pegol (Izervay) — recently FDA-approved for geographic atrophy (a late form of dry AMD); slow progression but don't restore vision
- Several other treatments in clinical trials
Treatment for Wet AMD
Anti-VEGF Injections (Standard of Care)
Intravitreal injections of anti-vascular endothelial growth factor agents have transformed wet AMD outcomes:
- Ranibizumab (Lucentis)
- Aflibercept (Eylea, Eylea HD)
- Bevacizumab (Avastin) — used off-label
- Brolucizumab (Beovu)
- Faricimab (Vabysmo)
These injections:
- Block the abnormal blood vessel growth
- Reduce fluid leakage
- Often stabilize or improve vision
- Typically given monthly initially, then less frequently with treat-and-extend protocols
- Performed in the ophthalmologist's office under sterile technique
Before these injections were available (early 2000s), wet AMD typically progressed rapidly to severe vision loss. Now, most patients maintain vision and many improve.
Other Treatments
- Photodynamic therapy — used much less commonly now
- Laser photocoagulation — limited use
- Combination approaches in selected cases
Practical Strategies for Living With AMD
For patients with established AMD:
- Use Amsler grid daily to detect changes
- Report new distortion or vision changes promptly (could indicate wet AMD development)
- Use better lighting
- Magnification devices (handheld magnifiers, electronic magnifiers, specialized software)
- Audiobooks, large-print books, and screen readers
- Adaptive technology for daily activities
- Low-vision specialty services
- Driving evaluation if vision affects safety
- Mental health support — vision loss is psychologically significant
Special Considerations
Patients with One Eye Affected
- Fellow eye risk is significantly elevated
- AREDS2 supplements warranted
- Aggressive risk factor modification
- Frequent monitoring of fellow eye
- Patient education on warning signs
Family Members
- First-degree relatives of AMD patients have substantially elevated risk
- Should establish ophthalmologic care
- Should optimize modifiable risk factors
- Annual dilated exams typically recommended starting age 50
Patients Who Smoke
- Smoking cessation is the most important intervention

- Risk of AMD progression remains elevated even after quitting but decreases over time
When to See an Ophthalmologist
Routine
- Baseline dilated eye exam at age 40
- Every 2–4 years before age 55
- Every 1–3 years from 55–64 (more often with risk factors)
- Annually after age 65
- Any patient with diagnosed AMD or significant risk factors
Urgent
- New distortion of straight lines
- New dark spot in central vision
- Sudden decrease in central vision
- Any sudden vision change
These could indicate progression from dry to wet AMD, where prompt anti-VEGF injection can preserve vision.
When to See Your Primary Care Doctor
- Family history of AMD
- Smoking and need for cessation support
- Cardiovascular risk factor optimization
- Discussion of supplements and overall health
- Coordination of multiple chronic conditions
