Drug Interactions
Ciprofloxacin is a moderate inhibitor of CYP1A2 and chelates with polyvalent cations, leading to several common but preventable interactions. It also carries class-wide risks including QT prolongation, tendon rupture, and CNS effects.
- Polyvalent cation products (calcium, magnesium, aluminum antacids; iron and zinc supplements; sucralfate; dairy; multivitamins with minerals): Form insoluble complexes that drastically reduce ciprofloxacin absorption. Take ciprofloxacin at least 2 hours before or 6 hours after these products.
- Theophylline: Ciprofloxacin inhibits CYP1A2 metabolism of theophylline, raising serum levels and risk of seizures and arrhythmia. Avoid combination if possible; otherwise reduce theophylline dose and monitor levels.
- Warfarin: Enhances anticoagulant effect through multiple mechanisms. Monitor INR frequently during therapy and for 1–2 weeks after stopping ciprofloxacin.
- Tizanidine: Co-administration is contraindicated due to dramatically increased tizanidine exposure causing severe hypotension and sedation.
- QT-prolonging agents (e.g., sotalol, amiodarone, certain antipsychotics, ondansetron): Additive risk of torsades de pointes. Avoid combination in patients with prolonged baseline QT or other risk factors.
- Corticosteroids: Concurrent systemic corticosteroid use, particularly in adults over 60, increases the risk of tendon rupture (especially Achilles tendon).

It is highly effective and orally bioavailable — and it carries a set of serious adverse effects substantial enough that regulators have restricted its use for common infections.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes that manage DNA supercoiling during replication. Blocking them causes double-strand breaks and rapid bacterial death, so unlike the bacteriostatic macrolides and tetracyclines, fluoroquinolones are bactericidal.
Ciprofloxacin has strong Gram-negative activity including Pseudomonas aeruginosa, which few oral agents cover, and this is its most valuable property. Its Gram-positive activity is comparatively weak — notably against Streptococcus pneumoniae — which is why levofloxacin rather than ciprofloxacin is the respiratory fluoroquinolone.
Oral bioavailability approaches that of the intravenous form, allowing serious infections to be treated orally that would otherwise need admission. That is genuinely useful and is a large part of why the class became so widely used.
Resistance has risen sharply with that use, particularly among E. coli causing urinary infection, to the point where empiric ciprofloxacin can no longer be assumed effective in many regions.
The Safety Restrictions
The FDA has issued multiple boxed warnings for fluoroquinolones, and they are the central fact about this drug class.
Tendinitis and tendon rupture, most often of the Achilles, can occur during treatment or months afterwards. Risk is substantially higher in patients over 60, in those taking corticosteroids such as prednisone, and in organ transplant recipients.
Peripheral neuropathy can begin rapidly after starting and may be permanent. Central nervous system effects include confusion, agitation, insomnia, and, less commonly, hallucinations and seizures — and in older patients these are frequently attributed to infection or delirium rather than to the antibiotic.
Aortic aneurysm and dissection risk is increased, which matters specifically in patients with existing aneurysm, Marfan syndrome, or significant atherosclerosis. Fluoroquinolones can also exacerbate myasthenia gravis.
Because of these, the FDA advises against fluoroquinolones for uncomplicated urinary tract infection, acute sinusitis, and acute bronchitis when other options exist — the risks exceed the benefits for infections that are self-limiting or treatable with safer agents. Clinical Use
Ciprofloxacin remains appropriate where its spectrum is genuinely needed: complicated urinary infection and pyelonephritis where culture supports it, Pseudomonas infection, bone and joint infection, certain intra-abdominal infections in combination, traveller's diarrhoea from susceptible organisms, and prophylaxis in specific exposures.
For uncomplicated cystitis it should generally not be used. Nitrofurantoin, trimethoprim-sulfamethoxazole, and fosfomycin are preferred, being effective, safer, and less damaging to the resistance landscape. Absorption is markedly reduced by calcium, magnesium, iron, zinc, and aluminium — dairy, antacids, supplements, and multivitamins must be separated by at least two hours before or six hours after. This is a frequent cause of treatment failure that looks like resistance.
Monitoring and Follow-Up
Short courses need no routine laboratory monitoring in healthy patients. Culture and sensitivity results should be reviewed and therapy narrowed or changed accordingly — ciprofloxacin is frequently continued empirically when a culture has already shown a safer agent would work.
Ciprofloxacin inhibits CYP1A2, which raises levels of theophylline, tizanidine, and caffeine, and it interacts with warfarin, raising INR. It prolongs the QT interval, and combining it with other QT-prolonging drugs warrants attention to potassium and magnesium.

Duration has shortened considerably as evidence has accumulated, and this matters more with fluoroquinolones than with safer classes because adverse effects accrue with exposure. Complicated urinary infection and pyelonephritis are now treated for materially shorter courses than was once standard, with equivalent cure rates. A patient given a two-week course where a shorter one would do is carrying additional tendon, neuropathy, and C. difficile risk for no benefit. Reviewing culture results and narrowing to a safer agent partway through a course is also legitimate and underused — the initial empiric choice does not have to be the whole course.
Special Populations
In older adults, tendon rupture, CNS effects, aortic risk, and hypoglycemia are all substantially more likely, and a safer alternative should be sought wherever possible. Concurrent corticosteroids compound tendon risk considerably.
In pregnancy fluoroquinolones are generally avoided. In children they are reserved for specific situations where no alternative exists, given effects on developing cartilage in animal studies. Kidney impairment requires dose adjustment. Patients with myasthenia gravis, known aortic aneurysm, or prior fluoroquinolone-associated tendon injury should not receive them.
Topical fluoroquinolone preparations are a separate matter and should not be confused with oral therapy. Ciprofloxacin eye and ear drops deliver the drug locally with negligible systemic absorption, and the boxed warnings that govern oral use do not apply to them in any practical sense. A patient told to avoid fluoroquinolones after a tendon problem can still generally use ciprofloxacin ear drops for otitis externa.
When to Contact Your Doctor
Stop the drug and seek advice for new pain, swelling, or tenderness in a tendon — particularly the back of the ankle — since continuing raises rupture risk. Report new numbness, tingling, burning, or weakness in the hands or feet promptly, as early discontinuation improves the chance of recovery.
Seek urgent care for sudden severe chest, abdominal, or back pain, which can indicate aortic dissection. Report confusion, agitation, hallucinations, severe insomnia, or mood change. Palpitations or fainting warrant evaluation, as does severe or persistent watery diarrhoea. Symptoms of low blood sugar need immediate treatment and review.
