A Disease Without Warning

Glaucoma is the second leading cause of irreversible blindness worldwide and the leading cause among certain populations. Roughly 3 million Americans have glaucoma, and an estimated half of them don't know it. The reason: most forms of glaucoma cause no symptoms until significant peripheral vision has been lost — and lost vision cannot be recovered.

The good news: glaucoma can usually be detected on routine eye examination years before symptoms develop. With appropriate treatment, vision loss can be slowed or stopped. The challenge is that detection requires regular examinations, particularly for people at higher risk.

What Glaucoma Is

Glaucoma refers to a group of diseases that damage the optic nerve, which connects the eye to the brain. Most cases are associated with elevated intraocular pressure (IOP), but some patients develop optic nerve damage at normal pressures.

The optic nerve damage:

  • Begins peripherally and progresses centrally
  • Causes characteristic patterns of vision loss
  • Is largely irreversible once it occurs
  • Can usually be slowed or stopped with appropriate treatment

Types of Glaucoma

Primary Open-Angle Glaucoma

  • The most common type (90 percent of US cases)
  • Gradual, painless vision loss
  • Usually bilateral but often asymmetric
  • Drainage angle of the eye is open but functions inadequately
  • Often elevated IOP, sometimes normal

Normal-Tension Glaucoma

  • Optic nerve damage and visual field loss with normal IOP
  • More common than once thought
  • Mechanism less clear; may involve vascular factors
  • Treatment still focuses on lowering pressure

Angle-Closure Glaucoma

  • Less common in the US but more common in some Asian populations
  • Acute angle closure is a true emergency
  • Sudden severe eye pain, headache, nausea, vomiting, blurred vision, halos around lights
  • Requires immediate ophthalmologic intervention to prevent permanent vision loss

Secondary Glaucomas

  • Caused by other eye conditions: trauma, inflammation, certain medications (especially steroids), pseudoexfoliation, pigment dispersion
  • Treatment includes addressing the underlying cause

Congenital and Childhood Glaucomas

  • Less common but require specialized pediatric ophthalmology care

Recognizing Glaucoma

The challenge: most glaucoma is silent.

Open-Angle Glaucoma

  • No symptoms in early or moderate disease
  • Late symptoms: peripheral vision loss, "tunnel vision," difficulty driving (particularly at night)
  • Patient often unaware until detected on examination

Angle-Closure Glaucoma (Emergency)

  • Severe eye pain
  • Severe headache
  • Nausea and vomiting
  • Blurred vision
  • Halos around lights
  • Eye redness
  • Pupil mid-dilated and non-reactive

This combination requires emergency department evaluation.

Who Is at Risk

Strong Risk Factors

  • Age — risk rises substantially after 60; over 5 percent of those over 70
  • Family history — first-degree relative significantly raises risk
  • African or Hispanic ancestry — significantly higher risk and earlier onset
  • Asian ancestry — higher risk of angle-closure types
  • Elevated IOP — though not all elevated IOP becomes glaucoma
  • Thin central corneal thickness
  • Optic nerve characteristics noted on examination

Other Risk Factors

  • High myopia (nearsightedness) — especially for open-angle
  • High hyperopia (farsightedness) — especially for angle-closure
  • Diabetes
  • Long-term steroid use (oral or topical)
  • Eye trauma history
  • Certain eye conditions (pseudoexfoliation syndrome, pigment dispersion syndrome)
  • Sleep apnea (emerging association)

Screening Recommendations

The American Academy of Ophthalmology recommends:

  • Comprehensive eye exam every 5–10 years before age 40
  • Every 2–4 years from 40–54
  • Every 1–3 years from 55–64
  • Every 1–2 years from 65 and older
  • More frequent screening for higher-risk individuals (African American patients, family history, high IOP, etc.)

For patients at higher risk, screening should start earlier and be more frequent.

Diagnostic Evaluation

Comprehensive glaucoma evaluation includes:

  • Visual acuity testing
  • Tonometry (IOP measurement)
  • Pachymetry (corneal thickness measurement, which affects IOP interpretation)
  • Gonioscopy (examination of the drainage angle)
  • Dilated examination of the optic nerve
  • Visual field testing (perimetry)
  • Optical coherence tomography (OCT) of the optic nerve and retinal nerve fiber layer
  • Sometimes additional imaging

The combination of optic nerve appearance, OCT findings, and visual field testing is more sensitive than any single test.

Treatment Goals

The goal of glaucoma treatment is to lower IOP enough to slow or stop optic nerve damage. Treatment is usually lifelong.

The target IOP varies based on:

  • Severity of existing damage
  • Rate of progression
  • Patient's life expectancy
  • Tolerability of treatment

Generally, IOP reduction of 20–30 percent or more from baseline is sought.

Treatment Options

Topical Medications (Eye Drops)

The mainstay of treatment:

  • Prostaglandin analogs (latanoprost, travoprost, bimatoprost, tafluprost) — first-line for most patients; once-daily dosing; side effects include eyelash growth, iris darkening, periocular skin changes
  • Beta blockers (timolol) — long-established; cardiac and pulmonary cautions
  • Alpha-adrenergic agonists (brimonidine)
  • Carbonic anhydrase inhibitors (dorzolamide, brinzolamide)
  • Rho kinase inhibitors (netarsudil) — newer class
  • Combination drops for patients needing multiple medications

Adherence to drops is challenging — over half of glaucoma patients don't take their drops as prescribed. This is a major reason for treatment failure.

Laser Treatment

  • Selective laser trabeculoplasty (SLT) — increasingly used as first-line treatment in some patients; treats the drainage angle to improve outflow; effects last years; can be repeated
  • Laser peripheral iridotomy — creates a small opening in the iris to prevent angle closure
  • Cyclophotocoagulation — for refractory cases

The LiGHT trial showed SLT as primary therapy reduces or eliminates the need for drops in many patients with mild to moderate open-angle glaucoma.

Minimally Invasive Glaucoma Surgery (MIGS)

A growing category of procedures:

  • iStent, Hydrus microstent, Xen gel stent, others
  • Usually performed at the time of cataract surgery
  • Lower risk profile than traditional glaucoma surgery
  • Modest IOP reduction
  • Particularly useful for patients with mild to moderate disease

Traditional Glaucoma Surgery

  • Trabeculectomy — creates an alternative drainage pathway
  • Glaucoma drainage devices (Ahmed valve, Baerveldt implant)
  • More effective IOP lowering but higher risks
  • Reserved for patients who haven't responded adequately to other treatments

What Doesn't Reverse Glaucoma

Important truth: glaucoma damage is largely irreversible. Treatment slows or stops progression but doesn't restore lost vision. This is why:

  • Early detection matters enormously
  • Adherence to treatment matters
  • Regular monitoring matters
  • Vision lost from delayed treatment usually cannot be recovered

Lifestyle Considerations

Some lifestyle factors may modestly affect glaucoma:

  • Aerobic exercise — may slightly lower IOP and is associated with better outcomes
  • Avoid head-down positions for prolonged periods (some yoga inversions, certain exercises)
  • Address sleep apnea when present
  • Caffeine in moderation is fine; very high intake transiently raises IOP
  • Marijuana lowers IOP transiently but the duration is too short to be clinically useful
  • Smoking does not directly affect glaucoma but worsens overall eye health

Lifestyle measures don't replace medical treatment.

Considerations for Specific Populations

African American Patients

  • Significantly higher risk of glaucoma
  • Earlier onset
  • More aggressive disease
  • Earlier and more frequent screening recommended (starting age 40 or earlier with family history)

Family Members of Patients with Glaucoma

  • Substantially elevated risk
  • Should establish eye care
  • Earlier and more frequent screening

Patients on Chronic Steroids

  • Topical, oral, inhaled, and even intra-articular steroids can raise IOP
  • Periodic eye monitoring during long-term steroid use
  • Address with prescribing physician when relevant

What Patients Should Know

  • Glaucoma is silent — you cannot tell if you have it without examination
  • Family history matters significantly
  • African American and Hispanic patients are at higher risk
  • Treatment is highly effective at preventing progression but doesn't reverse damage
  • Adherence to drops or other treatment is critical
  • Modern surgical and laser options reduce dependence on drops
  • Routine eye examinations are essential, particularly after age 40

When to See Your Doctor

Urgent (Same Day or Emergency)

  • Sudden severe eye pain with nausea and vomiting (possible angle-closure)
  • Sudden vision loss
  • Halos around lights with eye pain

Routine

  • Establishing eye care if you haven't had a recent comprehensive exam
  • Family history of glaucoma
  • Discussions about screening based on risk factors
  • Coordination of multiple medications affecting IOP
