Mechanism of Action
Methylphenidate blocks reuptake of dopamine and norepinephrine by inhibiting their transporters, increasing synaptic concentrations of both in the prefrontal cortex and striatum.
The prefrontal cortex governs executive function — sustained attention, working memory, impulse control, and task-switching — and in ADHD this circuitry appears to be underactivated rather than overactivated. This is why a stimulant improves rather than worsens attention in ADHD, a point that continues to confuse patients and families who expect a stimulant to make an already restless child more restless.
Methylphenidate differs mechanistically from the amphetamines, which not only block reuptake but also actively drive release of stored catecholamines from presynaptic vesicles. That difference in mechanism translates into a difference in response between individuals: roughly a third of patients respond better to one class than the other, and failing methylphenidate does not predict failing lisdexamfetamine or another amphetamine. Trying the other class before concluding stimulants do not work is standard practice and frequently succeeds.
